Swelling, : Swelling, pain, and bone spicules, and even an infection are complications of this type of extraction. You did not specifically ask a question, but the advice is to go back to your surgeon for treatment.

I : I would be suspicious that you had some foreign body in the problem extraction socket. Since you already had one post operative procedure, if you were my patient, i would open the extraction site completely, clean it out, and re-suture the site. Though not consider standard of care, i would also consider placing some platelet rich plasma in the extraction site to help with the healing (not all dentists have this capability though). I would also place you on antibiotics during the healing process..

Bony sequestrum: Sounds like a piece of bone became broken off, later the surgeon used a bone file to smooth the sharp end. It could be that the sharp end is a spicule of bone no longer attached and is non-vital called a bony sequestrum. It will continue to work its way out, much a like a splinter would in your finger. Go back to your oral surgeon.

See oral surgeon: Sometimes this happens. You may have another bony spicule that requires exploration again. Go back and see the surgeon again. If you have lost confidence, get another opinion.
