Typically : Typically when one has failed maximal medical therapy, and your history and exam is consistent with chronic sinusitis, the next step is to obtain a ct scan of the sinuses to see what the extent of the disease is, and if there are any anatomical factors that are predisposing you to these chronic symptoms. Only then can a determination be made as to the appropriateness of sinus surgery. If you haven't already, i would see an ENT physician and discuss these concerns.
