The : The most common reasons a nose would be broke would be to realign a malaligned boney nasal pyramid (crooked boney portion), to narrow excessively wide nasal bones, or to close an open roof deformity after nasal hump reduction. If there is minimal bridge excess sometimes it can be conservatively filed down without need to break the bones. If just tip work is needed there is no reason to break the bones.

Not always: Breaking the nose usually refers to "osteotomies" or the controlled cutting of the nasal bones. This is done to the upper third of the nose when the nasal bones need to be moved in some fashion. The most common reason the bones need to be moved is when the nose is crooked, or when the bones need to be narrowed.
