Have an evaluation: It sounds like depression is the major issue here and it can contribute to lack of sleep, decreased motivation, and other symptoms. Help is available and can be very beneficial. A combination of medication and counseling is usually a good course of action to start with. Your primary doctor is often a good a safe place to start.

Seek therapy: Find a good therapist and first find out why you are depressed then work on coping skills to deal with whatever is bothering you. Therapy is quite effective and you can feel better. Everyone needs help once in a while. It is the smart thing to do.
