You : You do not mention in your question how old you are but i would strongly encourage you to visit your gynecologist or primary care doctor. It can be ok to have spotting 18 months after your last period but it does depend upon several important factors such as your age. It is also important to know when your last pap smear was and whether you have any history of abnormal pap smears. Finally, if you are overweight then this does increase your risk of having abnormalities involving the lining of the uterus called the endometrium. Also, you need to be certain that the blood is indeed coming from the vagina/uterus and not the rectum. This question cannot be fully managed on-line. Please make an appointment to see your gynecologist or primary care doctor.

It : It may end up being something that is not serious or life threatening however, any vaginal bleeding after 1 year of no menses whether you are at menopausal age or not needs to be evaluated by a gyn doctor. Please do see a doctor as soon as possible. You will need a dxc or endometrial biopsy (sampling of the tissue in the uterus) as part of the work up. If the bleeding is coming from elsewhere, your doctor can help you figure that out and order the appropriate test for you.
