A member asked:

Can i do anything about my gummy smile? i've always been self concious of my smile, and am now wondering if there is any type of surgery that would correct it? would gum reshaping do the job, or is there something else out there i don't know about?

In : In reading your question, I am considering whether you feel your teeth are too small because the gingiva (gums) cover the necks of the teeth, making them look short, or if you are describing properly sized teeth, but that when you smile you see a lot of the gingival tissue above the teeth. The above are two different scenarios. If you feel the teeth are too small, then there is a procedure called a gingivoplasty/gingivectomy which can be done to give your teeth a longer, larger appearance. A periodontist would be a good place to start for an evaluation and consultation. If, however, you feel that you see too much gingival tissue above the teeth when you smile, that is more complex and is related to your maxilla and it's position & size in relationship to your lips. To make a change in that appearance would require a major surgical procedure by a team of surgeons including oral surgeons. A consultation with an oral surgeon in your area would advise you of the risks and benefits of such a surgery. I've tried to answer both cases and i hope I have helped.

There : There are procedures to eliminate or reduce a "gummy smile" in many cases. Sometimes they are relatively simple, but more often than not, they are extensive. You would need an evaluation from a competent dentist in order to evaluate your individual situation and discuss with you your options.

I : I would recommend a consult by a general dentist first to determine the cause of the gummy smile.Upon the results of an examination the dentist might refer you to an oral surgeon or a periodontist for an additional consult.

Determine the cause: Gummy smile can be attributed to several etiological factors. Firstly, teeth can hypoerupted, i.e., not fully erupted and therefore cover by gum. Gingivectomy or gum resection can treat this. Secondly, teeth fully erupted but with excess maxilla. Refer to oral surgeon for orthonagthic surgery. Thirdly, short philtrum. Plastic surgeon is a good consulting place. Finally, combine above factors.
