It : It is optimum to assess a patient in the context of their family. To interview both parents and the child/teen. It is also beneficial for the provider to observe how the family member's interact. However, children/ teen can also seek out assistance themselves. Many schools have counselors or psychologists that you can schedule an appointment with. Some of these providers have the diagnostic skills to diagnose depression. It would be extremely unlikely that as a child/ teen that you would be prescribed medication for a mental health problem without parental/ guardian consent. I hope this was helpful. Good luck on your school project.

Diagnosis yes: You can get a diagnosis of depression at any age with or without parent permission. You can not get treated for it however, without parent permission.

It depends: In some states it is possible to get not only diagnosis but also treatment as a minor without parental knowledge or consent. Usually this is based on a determination that it would be somehow detrimental to involve the parents. Since this is more of a medical/legal question, you might try contacting your state psychological association.

Usually no: If state law considers a person a minor and not an emancipated adult the diagnosis can be made with discussion with parent.

CHILD DEPRESSION: It is best for the child to meet with a Guidance Counselor at School to get help and referrals. The Counselor can guide the child to get help and make recommendations to the family.
