When : When thighs are evaluated and determined to have enough skin laxity to be considered for an inner thigh lift, the likelihood of having significant skin contraction is low. If one is borderline, the use of laser assisted or ultrasound assisted liposuction may provide enough extra contraction that a lift can be avoided. If one knows that a thigh lift is in their future, and there is sufficient recovery time, i would recommend doing it as a single procedure due to the need to perform extensive liposuction under the planned area of skin removal anyway. If the liposuction route is taken and the extra skin becomes bothersome, a thigh lift would become the likely needed solution, and in most circumstances only after months of recovery.

A : A thigh lift will address both the extra skin and the excess fat to achieve the tighter, thinner thigh. The trade off is that you gain a longer scar on the inside of your inner thighs. Liposuction is great at removing fat but not very good at dealing with extra skin. Even with laser liposuction, the "tightening" that is achieved is rarely more than an inch. So it is not very effective at dealing with "loose and flabby" thighs.

Liposuction and thigh lift is safe to combine in one procedure. You will likely get the best results, however, when you lose the weight that you want and remain stable at that weight for a few months.

Combined: Most plastic surgeons combine liposuction with a thigh lift to create the best contour. You cannot be too aggressive with the lipo to not affect the blood supply of the flaps. If you have severe wrinkling of the skin liposuction alone can make the appearance worse. Good luck.
