Yes : Yes if you have panniculitis and other symptoms such as rashes, incontinence, interference with daily living, etc. Consult with a board certified plastic surgeon.

Yes: Each insurer is different, but with a lot of weight loss and documented rashes, you may be able to get coverage. However, a panniculectomy doesn'tinclude the muscle tightening (plication) done with an abdominoplasty. You would have to pay out of pocket for that. Talk with a plastic surgeon for full details.
