Yes: Severe internal bleeding can cause significant strain on your heart and its function. It can be associated with lack of blood flow to the heart which can result in a myocardial infarction (heart attack).

Depends: Massive bleeding resulting in shock can lead to decreased coronary perfusion resulting in acute myocardial injury. Pts with pre-existing CAD are at higher risk. Lesser bleeding not resulting in hypotension is bettter tolerated by the pts myocardium.

Sometimes: If significant blood loss occurs causing a drop in blood pressure and you already have coronary artery disease a heart attack can occur. Without coronary artery disease sufficient blood loss can lead to cardiovascular collapse or what is termed hemorrhagic shock. If not corrected death can occur.
