COPD or asthma: It means air can get in but can't get out. It's usually a sign of airway obstruction, as is found in asthma, emphysema, or chronic bronchitis. Airways tend to open up as you breathe in, but as the lung exhales the airways collapse and air gets trapped in the distal air spaces.
