There is help: No individual diagnosis or advice can be given, but persisting paranoia and feelings are wathcing or talking about self gnerally raise questions about a range of disorders from paranoid personality disorder, delusional disorder, anxiety disorder etc. Ruling out medical conditions or medicaion effects and consulting with a psychiatrist asap would be wise. Early twenties can be risk time 4 worse.

A psychiatrist can help you, if you're willing to see one. S/he can explore with you what's happening, help find the cause, and suggest treatments that will help. You can start with your primary care physician first, but you may benefit from specialty care.
