Not painful: If performed by formally-trained surgeons, the procedure is not significantly painful. The is certainly some post-operative discomfort, but often not significant compared to other dental procedures, such as root canal therapy, etc.

Synthetic Roots: A dental implant is a specially fabricated titanium cylinder designed to replace the root of a natural tooth. It can be placed w/o pain when a tooth is extracted or after healing. It takes 3-4 months for it to fuse to one's jaw bone. At the time of osteo-integration, an implant can than be used to replace a single tooth or multiple implants can be used to anchor a segment or even an arch of teeth.

Titanium roots: You might think of them as titanium roots, replacing missing tooth roots. They are titanium cylinders with threads (like a screw) that are surgically placed into the bone.
