Bunions: Bunions can be painful at times and painless at other times. Increased pressure, shoewear and pressure on the lesser toes can cause pain.

Maybe you have gout: Gout commonly occurs at the same site of a bunion. Or you can have a bursitis. They can be painful but i don't think a patient of mine ever complained of "incredibly painful" unless they had gout.

Since pain is : Subjective, what is incredibly painful is moderately painful to another. Bunions sometimes don't hurt at all. They can be quite painful too. As was mentioned elsewhere, severe pain, swelling and redness can be a sign of an acute gout attack.
