What could cause constant throbbing pain in a tooth that had a root canal and crown years ago?

Many things: Most likely to be an infection or a fracture. In the upper jaw a sinus infection can also be suspected to be the cause. Clinical exam and radiographic evaluation is advised. See your dentist.

Failure: Unfortunately, even the best root canals and crowns eventually fail. Some may fail within months, while others may take decades. It is likely that your root canal is re-infected. If it is, a root canal re-treatment may be possible to save this tooth. Other possible causes are root fracture, gum infection, a problem with an adjacent tooth, or bite disbalance.
