Well looking for inflammation in brain, should mri be with or without contrast? could without contrast miss inflammation?

Yes, could miss: Sounds like a question of a diagnosis. The contrast is useful to locate a new lesion, where there is acute inflammation, as a result of a local breakdown of the brain-blood-barrier. For example, in ms, we can distinguish new vrs older lesions, with the contrast. Caveat, do not use contrast, if you have kidney problems.

BRAIN INFLAMMATION: Mri with contrast is better at visualizing any type of brain infection, vasculitis or inflammatory process.
