Some concerns: Your age of 23 suggests a number of possibilities: 1) irritable bowel syndrome; 2) adult hirshprung's disease or rectal dyssynergia with pelvic floor dysfunction; 3) relatively acute procto-colitis due to infection, inflammatory bowel disease, etc.; 4) ano-rectal disorders (like anal fissures, rectal prolapse, large prolapsing hemorrhoids with pain); 5) neurologic damage.
