Many things.: Could be something ( infection, tumor, inflammation, blood clot, etc. ) in the lung or against the lining of the lung at either the top of lung or against the diaphragm. Could be a muscle exterior to the chest wall or even in the chest wall. Unlikely to be something related to the rib directly without an obvious trauma.
