Many things: Could be anxiety since Ativan (lorazepam) seems to help it... But an evaluation by a cardiologist like you suggest sounds like a good idea given chest pains you are experiencing. Symptoms like these deserve a full evaluation in person by your health care provider.

If heart is ok...: Once heart is determined to be ok then a thorough exam of the shoulder should be done. An MRI of the shoulder to rule out ligament/tendon, rotator cuff etc. These are not picked up on by xrays. An EMG and nerve conduction study of the arm may pick up on a pinched nerve as well. Ativan (lorazepam) is a form of valium and can help with musle spasm and anxiety from pain.
