Diagnostic Testing: A combination of various x-rays and pulmonary function testing, including perhaps what is called a methacholine challenge test will help make a definitive diagnosis. Be sure to consult with a pulmonologist.

DLCO Is decr in emph: Besides significant reversibility with asthma, emphysema will have a low dlco. Emphysema pts will have a smoking hx. Asthmatics often have a family hx and a h/o allergic rhinitis. When asthma is not doing well it will wake the pt up at ~3 am give or take an hour. Also a good choice for a consult is an allergist immunologist-they-have a lot of experience and expertise in asthma.
