Dialysis/transplant: Acute and treatable causes to be addressed first. Creat 6.2 is advanced if it is all chronic. He needs to see a nephrologist if not already being followed. Options at that stage are dialysis and then transplant if possible. Clear understanding of available options is important before making any decision.

Unlikely: With a creatinine of 6.2 his kidneys are failing and not filtering the blood. Without dialysis he will become septic and could die. Looks to me as you really don't have a choice at least right now. If something acutely caused his cr to rise than treating that could keep him from needing dialysis chronically.
