Get urine culture: Return of symptoms might indicate that bacteria causing your UTI are resistant to Nitrofurantoin (active ingredient of macrobid) is not effective. Thus either original bacteria were not sensitive to the antibiotic, became resistant to it or you acquired a new or breakthru infection whil;st on macrobid. Thus u need a urine culture followed different antibiotic. Culture will reveal sensitivities.

Change antibiotics.: You likely have a resistant organism, insensitive to macrodantin (nitrofurantoin). Call your doctor; you should get a urine culture to determine the best antibiotic.
