Alter the load: If you are backpacking, be sure to optimize how you load your pack. Adjust the straps and keep the heavy items in the bottom of the back. Squeezing your shoulder blades together for 15-20 seconds several times/day can help off-load the upper trapezius which is usually the muscle that causes upper back tension. See a physiatrist for further recommendations.
