See Previous Answer: You basically have three options: resume opiates and taper gradually to minimize withdrawal. Use subutex (buprenorphine) as a replacement opiate which has less withdrawal issues. Use meds which block withdrawal symptoms like clonidine. The problem with the last option is that it can drastidcally lower your BP which might make your dialysis management difficult.
