Ineffective pumping: Your heart is failing as a pump, that is why your ejection fraction is only 38%. Ejection fraction means your heart is pushing out 38% of blood per beat. As heart pumps out less and less, it will try to compensate by beating faster, and you will have symptoms such as shortness of breath.

Not sure: Your diagnosis of "idiopathic" dcm means that the cause is not known. There are many good treatments for your condition, you may benefit from a medication change, change in diet, or change in exercise program.
