Unusual but possible: As we are taught in fellowship, "tumors don't read text books." supraclavicular nodal metastasis is a distant site, technically stage iv. Usually the pattern is regional spread followed by more distant spread, but sometimes there are "skip" situations. Best of luck.

Yes: Or there can just be subclinical involvement of the cervical nodes. Have them checked by an md.
