Read below: If the pain is below the medial malleolus, this could be tarsal tunnel syndrome. With the dorsiflexion pain you also have to rule out posterior or anterior tibial tendonitis or a pinched nerve to the area. I would see a podiatrist as this may be an easy resolution.

Probably need a X-ra: Impossible to say with any specificity but could be tendiniitis of the posterior tibial tendon, a chronic medial ankle sprain, a talar dome lesion on the bone of the ankle, or a stress fracture of the medial malleolus. Xrays and examination are needed to clarify.

Some possibilities: Include an inflammed nerve, posterior tibial tendonitis, neuritis of the superficial peroneal nerve or tendonitis of the tibialis anterior tendon. Try icing it, if no contraindication take antiinflammatories. If pain persists see a podiatrist or orthopod to give a more defined diagnosis based on clinical exam.
