A few things: Sharp pains in the toes can be due to arthritis, nerve irritation, injury, infection, ingrown nail, poor fitting shoes among many other things. You may need to see a podiatrist and get x-rays to narrow down the possibilities and start treatment.

Gout: Hot, painful and swollen toe is a typical picture of gout. You need to do the uric acid test. If it's negative other less common causes should be investigated.
