Ask neonatologist: Deciding whether your baby needs surfactant is more than just a yes/ no answer. A 27 weeker is deficient in this, so even if breathing well now, that may not be the case in 24-48 hours. If your neonatologist is recommending it, i would definitely go with it. It can prevent days to weeks of extra need for ventilation and works much better if given early rather than waiting to see if problems occur.

Clinical Picture?: This does depend on other clinical factors, but in general we would expect a 27 week baby on invasive ventilation to benefit from early surfactant therapy, yes.
