More like bellows: The lungs are suctioned to the rib cage. When the chest wall expands, the lungs are opened and air rushes in. When breathing out, the chest wall collapses and air is forced out.

Yes: The lungs contain multiple small units called alveoli where gas exchange takes place. With each breath, fresh air is brought into those units and all the carbon dioxide produced by the body is carried out on exhalation. The lungs do not completely empty on exhalation & there is still air left behind after each exhalation.
