Speech therapy: If you are having that much difficulty at age 3 understanding, your son needs to have a formal speech evaluation. This is done free where you live through your elementary school. Once kids hit age 3, the school systems are mandated by federal law to evaluate and provide therapy if needed for kids with suspected delays. Go to your local elementary school and advocate for him and get started asap!
