Conservative Rx 1st: Unless there is evidence of bowel or bladder incontinence, and groin numbness, you should start with non-surgical treatments, such as medications, physical therapy (swimming is fine), steroid injections. Surgery is considered only after failure to respond to the above treatments, and you have severe pain, weakness or numbness. Check with a surgeon to see if you need more aggressive treatment.
