Mcl sprain after dislocation of patella. recent lateral release , meniscus repair . how do they know no tear without mri? limp, popping noise , pain

Cannot be sure: An MRI is an appropriate imaging study to detect a ligamentous or meniscal tear. A physical exam by a physician can help distinguish between different injuries, but symptoms such popping noises wouldn't necessarily signify a tear or not.
