Multifactorial: While age, loss of elasticity, estrogen depletion, pregnancy and weight loss along with natural genetic inheritance are cited as a cause the greatest single contributor to sag from my research is sleep position. If your breasts are squeezed by your body against the bed surface they will eventually deform. Sleep on your back if and remember arms resting on them will still damage them.

Normal: The reduction in estrogen as one nears menopause can result in a reduction of the glandular tissue in the breast. This tissue often becomes fatty replaced. If weight loss occurs, the skin naturally will relax (sag). If wt gain occurs, the increased fatty content causes greater wt of the breasts also causing sagging. A breast lift might help.

Aging: It is all genetic predisposition. I have patients in their 40's who's breasts still look youthful and others who are deflated and saggy. When elasticity of the skin is reduced with aging the skin saggs from gravity and deflation is post pregnancy breast tissue atrophy. When conditions are mild breast implants are sufficient, if skin sagginess is significant some skin tightening surgery is need.

Breast sagging: Aging is the most common cause of breast sagging. Other causes include weight gain and loss, pregnancy, nursing, and genetics. Age/kids/weight: Sagging of breasts can occur with age, weight changes and post pregnancies/breast feeding. The latter factor also causes changes in the tummy area and has given rise to term "mommy makeover" for the procedures that are commonly performed to address these.

Factors involved probably include genetics, changes in weight, pregnancies, and general physical characteristics such as skin elasticity. “sagging” can be corrected with breast lifting; “plumping” can be achieved with the use of breast implants.
