Dental implant: I have not come across any occasion that medicaid has paid for dental implants. If lucky there might be coverage for denture or partial denture.

No: The pa state government has recently reduced medicaid dental benefits for adults to bare bones coverage! you may want to investigate u of penn or temple dental schools for discounted implant treatment. There are also several hospital based advanced training programs in the philadelphia region which might provide a partial subsidy for dental implant treatment. Good luck!

Rarely, if ever.: Medicaid coverage is state by state. While doubtful there is any coverage for dental implants due to periodontal disease or decay, there may be coverage for congenital defects like cleft palate. Medicare (federal) absolutely does not cover for dental implants for decay and gum disease. But then again, if it is a "medical deformity" maybe the medical side will pick it up.

No: Medicaid is federally funded, which means we all pay for it with our withholding tax. It tends to provide basic coverage, and although alternative treatment is less effective, it is less costly, and therefore more likely to be covered by a single payer tax-funded program.
