Vascular problems: You should be checked out for a condition called raynaud's phenomenon. The swelling of toes that turn red and hot intermittently could be an underlying small arterial vascular problem. Seek out a podiatrist or vascular surgeon for a more definitive evaluation.

See a rheumatologist: Gout is still one possibility. Sometimes the uric acid in the blood is falsely low during an attack. Although difficult to do from a small second toe joint, seeing crystals in joint fluid is the gold standard test for gout. You can also get a dual energy cat scan of the foot which may show gout. Other types of arthritis can mimic gout. Please see a rheumatologist.
