Observation v repair: Many hydroceles are small and asymptomatic and do not merit any treatment. Many are "reactive" and occur after trauma and surgery. These may resolve with time and observation only is prudent. If it is very large, or symptomatic, then surgical repair is usually performed, although needle drainage with or without sclerosis are used occasionally.
