Left fib cage pain: The left lower rib cage area contains stomach. Sleep and lungs. The bloating and gas suggests gastritis or peptic ulcer disease. Inflammation of pancreas is a possiblity. Does not appear to be pleurisy or pneumonia. Advise evaluation by a physician for physical exam and further work up.

Need to examine: It is not feasible to provide a meaningful opinion without additional history, physical examination and may be some tests. 23 lbs seems a little too much needs to be evaluated. It would be prudent to see your doctor. Wish you good health!
