See an ENT doctor: This is a long duration to have this occurring. I suggest you see an ear nose and throat doctor to evaluate the condition.

Malignancy: Viruses, cysts, malignancy and the like can cause raw areas on the skin or inside your nose as described. Sniffing illicit drugs and/or the like can also cause similar symptoms. Please call you physician or ear/nose specialist as soon as possible to diagnose your problem!

Vestibulitis: Can be associated with a dry nose, picking at the area, or using q-tips inside the nose. You can get a low grade bacterial colonization of the hairs inside the nasal skin(vestibule). This creates a chronic irritation, crustiness, and sometimes clear discharge with occasional blood in it. See an ENT for treatment, usually ointments and antibiotics. However, other problems need to be ruled out.
