Nothing: The second dose contains the same attenuated viruses as the 1st. It became recommended when it was found that the 'failure' rate with one shot was as high as 5% with measles protection.

No difference: It is the same vaccine given at 12 months old.A second dose is required to boost your immunity (higher protection rate) to these very contagious diseases with bad outcome when contracted.
