Yes: An extremely painful toothache could be from many reasons but one could be bacterial infection which the best treatment would be going to the doctors as you may need a prescription.

Nothing advisable: Tooth aches are a symptom that you need professional dental care. Don't treat yourself. Toothache=problem. Otc analgesics like tylenol, (acetaminophen) advil or Aspirin provide temporary pain relief only.

See Dentist: Take 1 regular Aspirin + 1 regular tylenol (acetaminophen) along with an antibiotic (like amoxicillin) and get to the dentist that can do extractions asap. They may suggest a root canal treatment or an extraction. However this infection that you have needs to be dealt with now or it will spread.

See a dentist asap: Make an appointment to see a dentist asap. Meanwhile, you can take an antibiotic if you have in the house like penicillin or amoxicillin, also you can take Motrin for pain. Most likey your tooth is infected and you may need a root canal in order to save your tooth.
