Yes: For blood clots anywhere, but particularly one in the brain, blood thinners prevent the formation additional clots, and facilitate the dissolution of the existing clots by your own healing mechanism. So, yes, someone can (ans should) definitely be released from a medical facility after a cerebral blood clot and placed on blood thinners.

Yes: Strokes are frequently blood clots. After stabilization and medication- anti coagulation patients may be transferred or released. Some other subdural or epidural clots are evacuated surgically and usually will not need anticoagulants.
