Combination drugs: There are many combinations that can be used, only if single drug does not do the job to bring aic hemoglobin to 6.5 or below which is the goal start with metformin and up the dose to max 2.5 grms a day, goal not reached add a sulphonlurea like Glipizide etc and max the dose, if goal not met add actose if you feel comfortable add ddp inhibiter like Onglyza (saxagliptin) or another combination are available as we.

Many options: The best combination will depend on ur clinical characteristics, insurance coverage, personal preference & tolerability. In general, the best combination would be something that does not cause weight gain/hypoglycemia & is well tolerated. From the arsenals we current have, i believe the combination that comes closest to the criteria above would be: metformin + glp-1 or metformin + dpp-iv inhibitor.
