Diabetes: Uncontrolled diabetes very often causes nerve damage, especially in the feet. Over time the high sugar in the blood causes damage to the tiny blood vessels that feed the nerves and then they are affected. Other complications from diabetes involve losing vision, kidney failure, heart disease. There are many treatments for diabetes that should help. Go to the doctor for an evaluation and treatment.

Get treated: What you are describing is possibly neuropathy caused by nerve damage from the diabetes. If he was diagnosed many years ago he should get treated. The disease is not going to go away by itself. He waits long enough and the effects will be permanent and spread upward. He's playing with fire by not getting this treated. At the very least, stop drinking sodas...

Many possibilities: Foot pain could be as a result of many different things. A thorough history and physical exam and possibly some laboratory and radiology exams are needed to determine the exact cause. Causes of pain may include: trauma (fracture, sprain, arthritis), infection, metabolic conditions (gout), musculoskeletal or biomechanical abnormality and neurologic conditions (neuropathy).
