Atypical: Your symptoms sound atypical and unlikely the heart. Young females (especially menstrating ) are unlikely to have coronary artery disease. The smoking is a risk factor, but without hx od type 1 diabetes or famial dyslipidemia, or a very syrong family history, the likely cause is non cardiac. A pinched nerve, anxiety or other cause should be saught.
