Pelvic mass: You need an accurate ultrasound because a mass which is solid is not often septated and cannot rupture if it's solid. Often final diagnosis of solid tumors requires tissue diagnosis.

Ruptured cyst: Sometimes a diagnosis is hard to make on ultrasound. Surgical diagnosis is the only way to know 100%. It could have been a functional cyst, an endometrioma, a dermoid, a cystadenoma. Unfortunately, it may not be answered based on imaging alone.

None: The only way to obtain an accurate diagnosis is with a tissue specimen. No blood test, ultrasound, imaging study, or exam can diagnosis an ovarian mass. With all of the above we can assess risks and determine the most likely type of tumor or cyst, but an accurate diagnosis requires tissue. Either a laparoscopic biopsy, cystectomy, or oophorectomy needs to be done if accurate diagnosis is required.
