Nerve issues: Numbness over the shoulder blade, about a silver dollar size can be seen in "notalgia paresthetica", but your problem could be a focal intercostal neuropathy, or even a referred issue from a thoracic disc. A neurological exam can assist diagnosis, and meds exist to control the symptoms.
