Maybe: I'd favor a bone island. Adult kidney cancer can do anything, including turning up in one bone years later. However, if the radiologist's first thought is "bone island", and you're comfortable with watchful waiting, and your physician is as well, the best thing might to be watch it. Truth is, i'd be quite surprised if it were metastatic cancer.

Possibly: Bone is a relatively common site of spread of kidney cancer. The odds that this is true in your case depend on the stage and grade of your original cancer and the radiographic appearance. If the radiologist described it as a bone island that makes it less likely that it is cancer. Discussing this with your doctor is a good idea, and the need for a biopsy or other further testing can be determined.
