Continuity?: Ask to an infectious disease specialist. Maybe continuity with the teeth and dental abscesses which are close to it. Other reason may be the structure of the bone at the jaw. Pts. On chemotherapy receiving medications for osteoporosis i.e. Alendronate have a particular weak structure of the bone there.
