Yes: If there is a lot of open skin then you can get a secondary bacterial infection. Also, you may want to make sure that your tetanus vaccine is up to date as well if you have gotten bit recently. Some spider bites (eg, brown recluse) may benefit from antibiotic treatment (a bit controversial).
